PHQ-9 and GAD-7, over time

The same two questionnaires a clinic uses, answered every two weeks, kept as a line you can read and bring to an appointment.

Check-ups asks the PHQ-9 and the GAD-7 when one is due, scores them the standard way, and shows each score beside the sleep, movement and days that were happening at the time. A loneliness check (UCLA-3) is there too, weekly, if you want it.

How it works

Asked when due

Each questionnaire asks about the last two weeks, so a new one is offered two weeks after the last. Skip it if today is not the day.

Scored the standard way

The same bands a clinic uses. The score is yours to read; nothing is called a diagnosis.

Item 9 on its own line

The PHQ-9’s ninth question is shown on its own, and the crisis lines are one tap away whenever it is answered above zero.

A page for your clinician

Beside the days

Six months of scores, made up

A line with a dip in July and the nights that went with it. The page for your clinician would show this, with the method underneath.
